You have to sign up on Joel's web site with a name, email address, and telephone number. The email address has to be one where you can receive email.

I would upload the Table of Contents but the attachment size limit is too restrictive.

It was an 8MB PDF file. I suppose your current limit is reasonable. 8MB isn't exactly the smallest file in the world.
