Akismat, my blog gets spammed this s**t out of by spammers, ever time i login there's like 100s of spam comments to delete :P
I use Akismet on my blog... It catches 99.9% of the spam. All other comments I manually review and decide whether to allow the comment, delete it, or flag it as spam.
Before enabling akismet spam was becoming a problem... but since, I rarely get comments from spammers. Most of my blogs comments end up being displayed.
I'm using auto-approve with WP-Spam free plugin which is working quite well. Akismet deletes too many legitimate comments. WP-Spam free takes care of most of the bots. So all I have to do is to delete the manual generated spam comments. I'm pretty lenient on the comments but the bogus comments get deleted.
I rarely check spam comments, only a few times a month.
Talking about spam comments, what is spam for you? is a comment spam if someone did a comment and write a keyword as his name, but drop an interesting comment that contributes to the conversation.
Or a real name but just saying "great post you did a good job!"

I receive many comments in one of my blogs. Akismet is on for sure but its never accurate, so when the spam queue goes above 30, i just scroll to see the user's names, if i see a real name i read the comment to decide if its spam or not.

Its because of spam that I do not allow comments on my blog. I disabled comments by default, and the templates are edited so the comments section have been removed.
Besides my blog, I also run a forum. When I post something on my blog, its linked back to a forum thread. If people want to post comments, they can sign up on the forum and post them there.
I use Askimet for on my blog and I empty the spam queue every day(without manual review) and than manually review all comments which have passed the Askimet protection.
