I don't really have one, I don't think a set in stone structure is necessarily a great idea either... as some offenses may be more serious then others. I'd say the best idea is to have a disciplinary procedure for each rule beause of the point I made in the last sentence.

I agree. I used to be pretty strict with my moderators, but now it doesn't bother me as much. Just as long as they log in and check out posts and stuff and remove any spam, that's fine and they're doing their job. They shouldn't have to be required to make posts in order to stay on the team. I find that rule alone pretty pointless.
I used to use mod handbook but not any more.
I have a set of rules for my mods to follow along with the member's rules. These are nothing regarding how to post, just some things to follow so they know what to do and not do. I do include a guide on how to handle various situations.
