Hey Zash, do you know how much traffic does xTendMedia required for sign-up? Thanks.

Not quite sure, I think they do want at least 100k uniques per month.

Mediashakers prevented people from paying for 2 months, I've heard many people complain that in many forums. Well Adversal is the best and I just got my first check from them.

I think adbrite is still better than smowtion. Smowtion counts very few impressions. I got accepted by Media Shakers yesterday and am very exited :P. Except this thread I have heard only good about them.

One of my proxies has good alexa. When I signed up I gave address of that proxy. I wrote in the description that I have 50 other such proxies and politely requested them to accept me as a publisher.
One of their affiliate managers emailed me and asked me to send them a full list of my domains, which I did. My account was approved the next day.
If you get rejected then remember keep trying apply again. They will approve you eventually.
