I've never really tried a "sales page" style proxy. But I do like to keep my url form 'below the fold' to make the user have to scroll to get to it. Keeping the form below the fold gives them more time to check out the rest of the page, ie, the ads and newsletter signup. I've had good success using this method.

Seems like a good idea. But the only way to find out for sure is to test it out, which is fairly easy if the theme is readily available somewhere or if you know enough coding to make the theme yourself.
