What do you think of following theme, i can get this coded asap, if you like. let me know i can make few custom themes for you free ..

Looks good.. for a proxy! Any suggestions for the advertisements could improve it though.

Just a thought...you could put advertisments above and below the address box; under the paragraph you could put a leaderboard or a combination of those would do fine.
