They are close, but not quite the same.
On-Page SEO consists of items such as:
- Title tag
- Description tag
- Heading tags (h1, h2, h3, h4)
- Alt tags on images
- etc...
On-Site SEO is more related to site design issues like:
- Sitemaps
- Linking structure
- PageRank sculpting

I never realize that there are 2 terms regarding this kind of SEO, I always believe that what Will mentioned above are on-site SEO...

Guess it really how you want to define/market your services. I never really offered a "on-page" seo package or service. I've always offered a "On-Site" SEO site package that included the "on-page" aspects mentioned above.

Well, it's "changed", but it's still there.
Google's algorithmic change made nofollow useless (and even slightly harmful) for PR sculpting, but you can still do:
White hat PR sculpting through site architecture
- Blach hat PR sculpting through redirected links
That's another one -- and also it's another one I really want to develop a good test for.
